Who has to complete the OSHA injury and illness recordkeeping forms?

Many employers with more than 10 employees are required to keep a record of serious work-related injuries and illnesses. (Certain low-risk industries are exempted.) Minor injuries requiring first aid only do not need to be recorded.

  • Employers with 10 or fewer employees throughout the previous calendar year do not need to complete these forms.
  • Employers with more than 10 employees or more anytime during the calendar year, may come under the requirement. Employers must keep in mind and include all full-time, part-time, temporary and seasonal workers in their calculations.
  • Employers with more than one location where the total employment number exceeds 10 when combined will all locations, in most instances the company must fill out the forms.
  • In addition to the small employer exemption, there is an exemption for establishments classified in certain industries like banks, medical offices etc. A complete list can be found on OSHA’s website.
  • Fatality and other serious event reporting as well as injury and illness surveys may involve

    other considerations.
