Incident Reporting

All occupational injuries, no matter how slight, must be reported to your Supervisor, Management and HSE immediately “Big or small report it all”

All injuries reported after the end of shift will be *subject to the injury being listed as non-occupational in which the employee will accept all costs associated with their injury. (* “subject to” will be determined by HSE and Mgmt. for what is reasonable)

All non-occupational injuries will require a CiMSA RTW form filled out by your physician stating you are released for full duty work.
